Scholarship Support Schedule Agreement Form

In order to provide effective support and to fulfill the requirements of the scholarship, regular contact with the Scholarship Coordinator, Sarah Mask is essential. Inability to complete check-ins and required documentation listed below could result in loss of scholarship. Please let Scholarship Coordinator know of any changes to contact information like phone number or address.

Please remember that the Scholarship Coordinator is a great support during your academic years and do not to hesitate to reach out for supports when needed.

Mid semester check-in each term by phone/virtual/in-person.

End of semester check-in after each term in-person/virtual.

After each academic year, you are required to send the following information to the Scholarship Coordinator in order to keep your scholarship:

1. Marks (15 days following the end of each term) 2. Yearly letter to our donor Ralph 3. Re-apply for Fred J. Watkins Scholarship Application
